Leaving hospital and can't return home?

if you are currently in hospital and do not have suitable accommodation to return to there are a number of services that may be able to help.

A number of hospitals in Leicestershire have access to housing workers (Hospital Housing Enablers) who can assist with your discharge and ensure that you have suitable accommodation to meet your needs. You should ask in the hospital whether there is any assistance they can provide.

If you have a home but it is no longer suitable for your needs you may be eligible for a Disabled Facilities Grant to make adaptations so that it does meet your needs. If you do not have anywhere to go that could meet your needs you should contact the Housing Choices Team on 0800 952 0079. We will provide advice and support to help you find suitable alternative accomodation.
