All premises with a licence to sell alcohol must appoint a Designated Premises Supervisor (DPS) who has a Personal Licence.
A Designated Premises Supervisor is an individual who holds a valid Personal Licence and is named on a Premise Licence which authorises the sale by retail of alcohol. The Premise Licence must specify the name, address and telephone number of the DPS.
Vary DPS
You can apply online to vary a DPS by completing the following forms:

- Vary DPS Consent Form - To be completed by the new DPS

If you are applying to Vary the DPS or Transfer the Premise Licence you must complete a Police Information Form found above. This form asks for further information to help the police process their part of the application. If you fail to supply the completed form then the police may object to the application.
Community premises
The committee of certain community run premises that have a Premise Licence under the Licensing Act 2003 can apply to have the mandatory condition requiring a Designated Premises Supervisor disapplied. To do this please complete the online form.
